Put NeoDement's pack in the "tf" directory, then put the KilliconInstaller.exe and "data" folder from the extra icons into the "tf" directory and hit the KilliconInstaller.exe.

Well, I just put everything in the tf folder. Do you have a killicon pack installed? I'm using NeoDement's, so maybe his killicon installer expects "motherfiles" to exist it can edit. If you don't have his mod, those files don't exist and the installer has nothing to edit. Maybe. Not sure how it works.
