Tried out the map, you can get to a certain point in the map the the game just crashes, weird.

There's the image again but with the path that the monster takes. I'm assuming those longer vents are safe for the player when the monster is nearby.

What causes the cave ins and the eerie sound getting louder? Is that the monster doing that?
The first two pictures that TheTailor25 posted caught me as odd.
Inside the map, there is a pipe set as a prop_physics. To be specific, the pipe was used in Half-Life 2 when you meet one of the rebels in the tunnels and he picks it up to fight off the manhacks. The pipe in this map however doesn't seem to have any special settings.
What makes it strange though is why it's even there, considering there's that bundle of beams blocking the way to get to it.

I don't think there is any "cave-in". However, the "monster" has some sounds attached to it that plays around the map. I'm guessing that if you were to take cover in one of the vents that's not marked with the monster's path, you should be able to even see it go by.
I'm uploading a video with a clear view of the whole map, hang on while it processes.
Please do this so I don't have to play it myself.
This is correct, just played it and the monster will only kill you if you're in it's way.
Ok now this freaks me the hell out.
If you make it to the end of the map, it takes you to a white room where it plays the sound called "taperedemption.wav"
The song plays for 40 seconds, and after the 40 seconds is done, it's instantly set to disconnect you from the map, back to the main menu.
If the monster catches you, it's supposed to do the "exit" command which explains why the game just quit to the desktop without warning.
Is this monster visible?

No, the monster isn't visible until you it "catches" you.
However, you can see some clear dustpuffs coming from the ceiling of the vent as it moves around the map.
White room - Heaven ? You escaped the Monster/Demon and gone to heaven.
It's a burned, half body.
It's just a standard HL2 corpse with no ragdolling, floating around the map.
Right, I think I figured that out as soon as the sounds were posted.
Conclusion is, I guess when allisquiet.wav is playing, it's perfectly safe to wander around.
When onthehunt.wav is playing, it's a good idea to find some cover out of the evil charred body's tracks. That music gets louder.
When tasteofblood.wav is playing... I guess you should see him coming by?
screamingdeath.wav is what I heard before my game crashed, which implied that I died.
And as slayer20 said, a visual presence is the cave-ins that he brought up.
For some reason that reminds me of gm_ghosthunt...
Oh well, this whole thing is creepy as fuck.
Maybe it's an ARG for a new gm_ghosthunt map?

The game doesn't crash, it's a command in game that exits it.
So has anyone actually made it to the end?
I loaded up the map and I don't see any corpse, just a steam cloud floating around the map with a sound attached to it. :/ when you make it to the end it teleports you to a room and plays a sound then returns you to the main menu.

I think you need to load it in Ep2 if you aren't already.

If the dust cloud catches up to you, the corpse appears and the game exits.

"escapedestination" has nothing to do with "heaven"
