Bigger maps...say 10x10km. I know I know...."hurrrr not possible in teh sourcez!", but similar results would be possible by creating a map built to a smaller scale, and an add-on that scaled everything else down; props, players, physics, textures... everything.
I think people have tried that, to drastic results.
It wasn't done properly then.
On that note, I'd LOVE to have the source engine support larger maps.
The olde Quake 3 engine did, why they downgraded is beyond me...

Someone used Euphoria with HL2 models, but it looked like shit and glitched everywhere.
I'd really like a realistic gib system for npcs/ragdolls, instead of the fucking "goremod" that just makes limbs explode and deflates them completely.
Also an update to Headspack, we have Gman and the Guerilla terrorist with all of their limbs but everyone else is just headless bodies.
A PCMod Upadte that made the computers, Upgradable, Mutiskined and, Being able to do more things altogether.

Something Similar to Bonemerge Tool, but allowing even more freedom. To The point of allowing head/limb switching and colouring.
Like an in-game addon to create a new skin for something. Like...a Character Creator like you see in fighting games.

Yeah, but making the most awesome, realistic and epic contraption would either require immeasurable skill in Wiremod, and a shitload of time. So that's the point in it.
OP's dream mod: we could write a disk for PCMod, it supports custom programs.
GoldSrc (later Source Engine) forked out from from the Quake 1 Engine and used bits of IDTech2 (Quake 2 Engine) code, for instance the GL renderer. No part of Quake 3 code is in HL2 to this day.

I'd mainly see this being useful for multiplayer since there is already a somewhat similar thing for Fallout, or if you are really into screwing around, use modern military weapons to rule or destroy skyrim.
Redstone is crap.
Something like KOHCTPYKTOP would kick ass though, doped silicon and building simple gates from transistors, and then using those gates to make more complex gates..

you can do this in PAC3 already
PAC3 is still WIP but it works and you can do pretty much everything PAC2 can plus more. It just lacks some "end user" stuff such as spawning with outfits, a better browser and some in game documentation.

True roleplay with dynamic player models, aging, very large maps, good players, hunger, thirst, sleep, yeah.
A mod that allows the player to make more advanced NPC behaviors and use custom skins/models using a user friendly interface. Like: a tool that allows you to dictate what the NPC should do in your own scripted event, NPCs can get into vehicles, set paths for NPCs (e.g. Make a citizen NPC walk around a city), right click on the NPC's icon to choose an alternate skin/model, specialized behavior for different weapons (e.g. If an NPC picks up a sniper, they won't stand in the middle of the battlefield shooting randomly without scoping in), make NPCs able to use custom sounds, TF2 NPCs (?), Counter Strike NPCs (?), More vehicle NPCs.

A Massive Citywide map, Mile for Mile Recreation of New York City.
Perfect for Builds, Massive areas for Flying around, and nice NPC Wars, Exploration would be a plus too.
A plugin that lets you access modeling programs from an in-game GUI and then compile them and place them in the spawn-list of your choosing. If done on online servers with it enabled, the files will be shared with everyone temporarily. There could be modeling servers, and the ability to create and share models so quickly while IN THE GAME would mean alot of things for the mod community.
